朝陽科技大學波錠紀念圖書館場地借用申請

    本申請表依個人資料保護法第8條規定向您告知下列事項：
  1. 填申請表之目的：為辦理借用圖書館場地申請，僅作為方便與使用申請單位借用場地相關事項之聯繫。
  2. 填寫個資之類別：識別類(C001)姓名、單位、電話、E-mail等個人資料。
  3. 利用期間、地區、對象及方式：本校將於校區內為本次圖書館場地借用目的利用您的資料。
  4. 個人資料之權利及權益：您得依個人資料保護法第3條就您的個人資料行使請求查閱、補充、更正、製給複製本及請求停止蒐集、處理、利用及請求刪除等權利，行使方式請洽本校圖書館借還書櫃台(電話:2332-3000 分機3151~3152)。
  5. 本校於蒐集您個人資料時，如您不予提供將無法完成借用場地申請。

    6. For the purpose of applying for the library resource usage and services, CYUT should collect your personal information including name, employee number, unit, cellphone number, phone number, residential address, e-mail address (C001). CYUT uses your personal information for reasonable usage from now until CYUT operating existence within Taiwan. Please provide correct complete information to finish the application procedure. You reserve the right under Article 3 of the Personal Data Protection Act to request a review, request to correct your personal information if necessary. If you need to exercise the right, please contact Reader Service Section (ext. 3142).
